Your First Visit

Your first visit is important to us. In order to make this visit a rewarding experience several items of information are needed for the visit. These items are listed below.

Also, we have made the forms for a new patient available to you to download and fill out at your convenience and to save you time during your first visit.

Please bring the following items to your first visit:

• Insurance Cards
  Please bring your insurance cards including Medicare/Medicaid cards.
• Drivers License or Photo Identification
• Medications You Are Taking
  A list of any medications you are currently taking (including eye drops). Remember to include over-the-counter medication.
• Glasses or Eyewear
  Any glasses that you have been wearing.
• Chief Complaint
  Knowledge of your chief complaint, and date problem began, etc.

• Medical History Form
• Patient and Billing Information Form
• Patient Consent Form
